The undersigned Trustee hereby designates the Proxy identified below as my proxy with respect to my position on the Board of Trustee of the Bay Village Rockets Association for the meeting on or about the date below. This designation of proxy shall be effective for said meeting and at all adjournments of such meeting, and if applicable. The Proxy shall have the full power, as my substitute, to represent me and vote on all issues and motions that are properly presented at the meeting(s) for which this designation of proxy is effective.
